I understand that volunteering as an In House, Driver, or Receptionist means committing to weekly or biweekly volunteer shifts for at least 6 months. I will do my best to honor that commitment. *

I authorize Ronald McDonald House Charities West Michigan to carry out a background check and that any applicable criminal conviction, such as unsatisfactory driving records for driving roles, may be cause to disqualify me as a volunteer. *
